肺心病患者血浆D-二聚体水平和肺动脉收缩压及二氧化碳分压的相关性分析

摘    要:
目的:探讨慢性肺源性心脏病患者不同水平血浆D-二聚体(D-dimer)肺动脉收缩压情况,以及高碳酸血症与血浆D-dimer水平的相关性。方法:对86例住院治疗的肺心病患者二氧化碳分压(PaCO2)、血浆D-dimer水平及肺动脉收缩压(SPAP)进行测定,按照血浆D-dimer水平的高低分组比较SPAP水平的差异性,同时对PaCO2与D-dimer水平进行相关性分析。结果:血浆D-dimer水平升高的肺心病患者,SPAP水平要高于血浆D-dimer水平正常的肺心病患者,二者之间差异有显著性。同时,PaCO2与血浆D-dimer水平呈显著正相关。结论:肺心病患者存在血液凝血状态异常,持续的肺部微小血栓形成是导致及加重肺动脉高压的一个因素。肺心病患者的高碳酸血症是参与血液凝血状态异常的因素之一。

Abstract:
Objective:To investigate the relationship of the level of SPAP under the different level of plasma D-dimer and the Correlation of D-dimer and Partial Pressure of CO2(PaCO2) in patients with Chronic pulmonary heart disease. Method:Plasma D-dimer level,SPAP and PaCO2 were measured in 86 patients with Chronic pulmonary heart disease. 86 patients were divided into tow groups: The plasma D-dimer level of patients in Group A was higher, and the plasma D-dimer level of patients in group B was normal. Results of SPAP in two groups were compared.Result:The level of SPAP in group A was apparently higher than that in group B(P〈0.05). Plasma D-dimer level was positively correlated with PaCO2 (r=0.241,P〈0.05). Conclusion:Abnormal coagulation state and embolization of tiny emboli may be related to the development of pulmonary hypertension. Hypercarbia may be contribute to one of the abnormal coagulation state.
