盲探插管装置引导经鼻气管插管抢救呼吸衰竭

目的研究盲探插管装置在呼吸衰竭病人经鼻气管插管的应用价值。方法呼吸科ICU和神经科ICU因呼吸衰竭需气管插管病人124例，直接采用盲探插管装置经鼻插管58例，传统的盲探或明视经鼻气管插管64例，因鼻道畸形气管切开2例。结果采用盲探插管装置的58例病人，一次插管成功率96％；传统盲探法64例病人，经3次试插成功23例，经窥喉明视协助插管成功35例，有6例插管困难改用盲探插管装置插管成功。结论盲探插管装置引导经鼻气管插管用于呼吸衰竭病人急救，能达到快速开通气道、减少病人损伤的目的，熟练操作者在病床一侧即能完成气管插管，值得推广应用。

Pernasal Tracheal Intubation Guided by Blind Tracheal Intubation Instrument to Rescue Respiratory Failure

Objective To study the clinical value of pernasal tracheal intubation guided by blind tracheal intubation instrument to rescue respiratory failure. Methods One hundred and twenty-four patients of respiratory failure in ICU who need pernasal tracheal intubation were divided into two groups. One group （ 58 patients） , pernasal tracheal intubation was guided by blind tracheal intubation instrument;the other group（64 patients） received traditional pernasal tracheal blind intubation technique. Results The rate of first attempt to success in group received blind tracheal intubation instrument was 96% （55/58） , only 23 patients with three attempts to success in group received traditional pernasal tracheal blind intubation technique ,35 patients under direct viscion by largngoscope,6 patients to success by blind tracheal intubation instrument at last. Conclusions Pernasal tracheal intubation guided by blind tracheal intubation instrument can elevate the intubation successful rate. It has clinical spread value.
