咪唑安定联用吗啡在新生儿机械通气中的临床疗效

目的:评价咪唑安定与吗啡联合使用在新生儿机械通气治疗中的临床疗效。方法:选择在新生儿重症监护病房(NICU)机械通气治疗的患儿,共45例,随机分为观察组(咪唑安定+吗啡)20例、对照组(咪唑安定)25例。观察两组治疗后2 h的PO2、PCO2、PIP、PEEP、呼吸、心率、平均动脉压之间的差异,并比较两组机械通气时间及咪唑安定的用量。结果:两组患儿都可以达到满意的镇静效果,观察组咪唑安定的用量、机械通气时间、PO2、PCO2与对照组比较有统计学差异,两组间的RR,HR,MAP比较,均无统计学差异。结论:咪唑安定与吗啡联合使用,可以达到良好的镇静效果,有利于改善氧合状态,减少咪唑安定的用量,缩短呼吸机使用时间。

Clinical curative effect of midazolam combined with morphine during mechanical ventilation

Objective:To investigate the sedative effect of midazolam combined with morphine during mechanical ventilation.Methods:45 children were treated with mechanical ventilation in neonatal intensive care unit(NICU).They were divided into midazolam combined with morphine(A) group(20 cases),and midazolam(B) group(25 cases).Then the numbers were statistically compared between group A and B.After treatment for 2 hours: the heart rate,breath rate,blood pressure,analysis of blood gas,PO2,PCO2 and PIP,PEEP were compared between group A and B.And the mechanical ventilation time and dosage of the midazolam between group A and B were statistically compared.Results:Children in two groups could achieve satisfactory sedative effect.The mechanical ventilation time,dosage of the midazolam,indexes of blood gas,such as PO2,PCO2 of group A were all improved compared with those of group B.The differences between two group were statistically significant.There were no significant differences in RR,HR,MAP between the two groups.Conclusion:Midazolam combined with morphine during ventilation treatment has obvious sedative effect,which will benefit to improve oxygenation,reduce the amount of midazolam,shorten the time of respirator use.
