第二代双源CT虚拟平扫在肾脏占位中的应用

摘    要:目的探讨第二代双源CT虚拟平扫(VNC)在肾脏占位性病变中的临床应用价值。方法对52例肾脏占位性病变的患者行常规CT平扫和双能量CT增强扫描。经liverVNC获得虚拟平扫(VNC)图像,由2名医师对VNC和常规平扫的图像质量及病灶显示情况进行评价,测量双肾病变区平均CT值、SNR及辐射剂量,并进行比较。结果VNC图像在病灶显示方面完全满足诊断要求,与常规平扫比较差异无统计学意义(P〉0.05),3例钙化灶在VNC显示较常规平扫体积缩小。所测量各部位VNC平均CT值与常规平扫差异均无统计学意义(P〉0.05);利用t检验对比分析VNC与常规平扫选在肾实质、腰大肌、主动脉及肾占位的图像,虚拟平扫(VNC)SNR低于常规平扫(P〈0.05);虚拟平扫单期CTDI、DLP、有效剂量ED分别为(12.64±3.74)mGy、(464.50±207.37)mGycm、(6.96±3.11)mSv,均低于常规平扫的(17.52±2.12)mGy、(640.94±195.26)mGycm、(9.16±2.92)mSv,差异均有统计学意义(t值分别为2.588、4.842、2.588,P均〈0.05)。结论对与肾脏占位性病变,第二代双源CT可提供高质量虚拟图像.并且将明显低辐射剂量。

Preliminary application of the second-generation dual-source CT virtual non-enhanced in renal masses

Abstract:Objective To assess the clinical application of virtual non-contrast the renal masses CT from the second- generation dual-energy. Methods Fifty-two patients suffering from renal masses disease were performed conventional plain scan and dual energy enhanced scans. VNC image were obtained through the liver VNC software, two physicians were compared the image quality and lesion's detestability of the VNC and conventional plain, measure the average CT value, SNR, radiation dose. Results CT value of the average liver in VNC image higher than conventional plain aver- age CT Value; Compared with conventional plain, CT value of the other parts in VNC image were not statistically sig- nificant (P 〈 0.05), the detestability of lesions were not statistical significance (P 〉 0.05); By using t-test comparison analysis selection ROI of the liver, renal parenchyma, psoas muscle, the aorta and renal masses in VNC image, SNR of VNC was higher than conventional unenhanced (P 〈 0.05); While VNC image quality conventional unenhanced was not statistically significant (P 〉 0.05); Compared with VNC imaging, the detestability of calcification in real plain is narrow, while the other lesions can be shown. The radiation dosage of VNC were slightly higher than conventional plain scan (P 〈 0.05). The virtual unenhanced single period CTDI, DLP effective doses ED were (12.64±3.74) mGy, (464.50±207.37) mGyem, the (6.96±3.11) mSv were lower than conventional plain of (17.52±2.12) mGy, (640.94± 195.26) mGycm, (9.16±2.92) mSv, the differences were statistically significant (t=2.588, 4.842, 2.588, P 〈 0.05). Conclusion The second-generation Dual-source CT enhance scan in the renal masses can provide high-quality virtu- al image, significantly reduce radiation doses.
