ARB与ACEI联合治疗血液透析患者慢性心力衰竭的临床疗效分析

目的探讨联合血管紧张素Ⅱ受体拮抗剂（ARB）和血管紧张素转化酶抑制剂（ACEI）治疗血液透析患者慢性心力衰竭的临床疗效。方法随机选取本院60例血液透析患者，均分为实验组和对照组，对照组每日服用福辛普利片治疗，实验组在此基础上服用缬沙坦，疗程均为半年，观察两组疗效及并发症情况。结果两组治疗前后左室射血分数以及心脏指数均有明显提高，但实验组较明显，两组治疗有效率分别为85．7％和70．4％。差异有统计学意义（P〈0．05）。两组死亡率分别为6．7％和10．0％，差异无统计学意义（P〉0．05）。结论血管紧张素Ⅱ受体拮抗剂联和血管紧张素转化酶抑制剂治疗血液透析患者慢性心力衰竭，疗效显著，并发症少，值得临床推广。

Clinical analysis of ARB and ACEI in treatment of hemodialysis patients with chronic heart failure

Objective To explore the clinical efficacy of angiotensin Ⅱ receptor blockers （ARB） combined with an- giotensin-converting enzyme inhibitor （ACEI） in hemodialysis patients with chronic heart failure. Methods Sixty cases of hemodialysis patients in our hospital were selected and divided into experimental group and control group, the con- trol group were given Fosinopril piece and the experimental group was given Valsartan on basis of control group. Effi- cacy and complications were observed after six months treatment. Result Left ventricular ejection fraction and cardiac index of two groups were significantly improved, the experimental group was more apparent, the treatment efficiency rate of two groups were 85.7% and 70.4% respectively, the results were statistically significant （P 〈 0.05）. The mortali- ty rates of two groups were 6.7% and 10.0%, the difference was not statistically significant （P 〉 0.05）. Conclusion Angiotensin Ⅱ receptor inhibitors combined with angiotensin-converting enzyme inhibitor in therapy for hemodialysis chronic heart failure patients has significant effect, fewer complications, and it is worthy of promotion.
