终末期肝病患者肝功能综合评价

目的通过对因病毒性肝炎及其相关疾病死亡病例的死因分析,探讨终末期肝病患者的肝功能状态评价方法,进而讨论肝功能衰竭的诊断标准。方法本院1995年4月～2007年4月间住院患者,最终因病毒性肝炎及其相关疾病死亡的病例78例,按诊断结果采用自然分组,分别对各组患者进行分析。结果终末期肝病患者的直接致死原因为上消化道出血33.33%,顽固性腹水19.23%,肝性脑病20.51%,肝肾综合征11.54%,严重感染11.54%。结论对慢性肝病患者的肝功能评价时,除要注重由肝实质细胞损害所引起的指标变化外,还应考虑肝纤维化增生所致门静脉高压所引起的一系列临床表现。

Comprehensive Observation on Liver Function of Patients with End-stage Liver Diseases

Objective To determine the liver function of patients with end-stage liver diseases,through analyzing the death causes of patients with virus hepatitis and its related diseases and to discuss the diagnotic criteria of liver failure.Method A total of 78 cases of end-stage liver diseases,who died of virus hepatitis and its related diseases in our hospital durng Apr.1995 to Apr.2007,were naturaly separated into 6 groups according to diagnostic results.The liver function of patients in each group was described.Results The direct death causes of end-stage liver diseases are upper digestive tract hemorrhage(33.33%),severe ascites (19.23%),hepatic encephalopathy(20.51%),hepato-renal syndrome(11.54%),and severe infection(11.54%).Conclusions When observing the liver function of patients with chronic liver diseases,the focus of our attention should be directed at the clinical manifestations caused by portal hypertension due to liver fibrosis hyperplasia,rather than the changes of indexes caused by liver parenchymal cell damage.
