脊柱术后急性感染的治疗策略探讨

目的：总结脊柱术后急性感染的临床表现，探讨临床治疗方法，评估疗效，为临床治疗提供参考依据。方法回顾性调查分析2006年1月～2011年1月本院脊柱手术患者的临床资料，42名患者发生手术部位急性感染，积极采用抗生素治疗，清创开放引流及持续封闭负压引流治疗，观察治疗效果。结果通过相关的治疗处理，42例患者均获得了临床治愈，无死亡病例，且均未拆除内置物，平均住院时间26 d（住院时间12～73 d），出院后随访24～36个月原有症状均未再次出现。结论手术部位急性感染是脊柱术后常见的并发症之一，及时采取有效干预措施，临床效果较好。

Study on treatment of early postoperative infections following spinal surgery

Objective To provide basis for the clinical treatment of early postoperative spinal infection by discussing clinical presentation and evaluating the management and its effect.Methods All patients treated by spinal surgery from January 2006 to January 2011were retrospectively analyzed. Early postoperative spinal infections were observed in 42 cases. Patients' presentation was reviewed. The efficiency of treatment including intravenous antibiotic therapy, debridement and vacuum sealing drainage was evaluated.Results After the implementation of relevant management, all the 42 cases were cured. Spinal instrumentation was preserved in all cases. The average length of hospital stay was 26 d (range 12-73 d). Follow-up averaged 30 months(range 24-36 months)and there was no patient with recurrent infection.Conclusion Early postoperative infection of surgery site is a common complication of spinal surgery. Aggressive, effective management can achieve excellent clinical effect.
