经伤椎椎弓根打压植骨治疗胸腰椎骨折临床研究

目的探讨经伤椎椎弓根打压植骨治疗胸腰椎骨折的临床效果。方法选取2010年11月至2013年5月收治的160例胸腰椎骨折患者,其中的80例患者采用钉棒内固定系统跨伤椎固定的治疗方法作为对照组,另80例患者采用钉棒系统结合经伤椎椎弓根打压植骨的治疗方法作为实验组。比较观察两组患者治疗后的临床疗效、并发症发生率和患者满意度。结果实验组患者经3个月、1年后的椎体压缩率(10.2%±8.6%,5.3%±5.5%)明显低于对照组(18.8%±11.8%,16.8%±10.5%),差异有统计学意义(P0.05);实验组并发症中出现断钉、螺丝松动发生率(1.25%,3.75%)明显低于对照组患者(11.25%,30.00%),差异有统计学意义(P0.05);实验组患者的满意度(98.75%)明显高于对照组(63.75%),差异有统计学意义(P0.05)。结论采用经伤椎椎弓根打压植骨的方法治疗治疗胸腰椎骨折,能有效恢复椎体高度,并发症少,并且治疗后患者满意度高,值得在临床上广泛应用。

Transpedicular bone grafting for treating thoracolumbar vertebral fracture

Objective To discuss the clinical efficacy of transpedicular bone grafting for treating thoracolumbar vertebral fracture. Meth. otis One hundred sixty cases of thoracolumbar vertebral fracture treated in our department from November 2010 to May 2013 were selected, 80 cases treated with screw - rod instrumentation system served as control group, 40 cases treated with transpedicular bone grafting served as experimental group. The clinical effect and incidence of complications of two groups were compared. Results Three months and one year after surgery, the vertebral compression rate were 10.2% ~ 8.6% and 5.3% ~ 5.5%, respectively in experimental group and they were significantly lower than con- trol group （ 18.8% ~ 11.8% and 16.8% ~ 10.5%, respectively）, with statistical significance （ P 〈 0.05 ）. Incidences of breaking nail （ 1. 25 % ） and flexible screw （ 3.75 % ） in experimental group were significantly lower than control group （ 11.25 % and 30.00%, respectively） （ all P 〈 O. 05 ）. The patients＂ satisfaction in experimental group （98.75 % ） was significantly higher than control group （63.75 % ） （ P 〈 O. 05 ）. Conclusion Transpedicular bone grafting has advantages of less complications and effective restoration of vertebral height for the treatment of thoracolumbar vertebral fracture, In addition, the patients are generally satisfied with the treatment outcomes.
