Cpap与Bipap应用于急性左心衰合并呼吸衰竭患者的疗效研究

目的探讨急性左心衰合并呼吸衰竭患者应用持续正压通气(Cpap)与双水平气道正压通气(Bipap)治疗的临床治疗效果。方法对比35例在常规治疗的基础上加用Cpap治疗的急性左心衰合并呼吸衰竭患者(Cpap组)与34例在常规治疗的基础上加用Bipap治疗的急性左心衰合并呼吸衰竭患者(Bipap组)治疗前后血压[收缩压(SBP)、舒张压(DBP)]、心率、呼吸及血气分析[氧分压(PaO2)、二氧化碳分压(PaCO2)]的改善情况。结果两组患者治疗前SBP、DBP、心率、呼吸及PaO2、PaCO2结果比较无明显差异,经治疗12 h后两组以上指标均明显改善,且Bipap组改善较Cpap组更为明显。结论在基础治疗的基础上加用Bipap治疗可显著提高急性左心衰合并呼吸衰竭患者的治疗效果。

Study on the efficacy of application of Cpap and Bipap in treatment of patients with acute left heart failure complicated with respiratory failure

Objective To explore the clinical efficacy of application of continuous positive airway pressure （CPAP） and bi - level positive airway pressure （BiPAP） in treatment of patients with acute left heart failure and respiratory failure. Methods A total of 35 patients with acute left heart failure and respiratory failure （ CPAP group） were treated with CPAP on the basis of conventional therapy and 34 patients with acute left heart failure and respiratory failure were treated with BiPAP on the basis of conventional therapy （ BiPAP group）, and blood pressure （ SBP, DBP）, heart rate, respiratory and blood gas analysis （ PaO2 , PaCO2 ） were compared before and after treatment in order to know the improvement in their situation. Results The difference in SBP, DBP, heart rate, respiration rate, PaO2 and PaCO2 between patients of these 2 groups before and after treatment was not significant, but above indexes at 12 hours after treatment were significantly improved, and patients in BiPAP group were more significantly improved than those of control group. Conclusion The application of BiPAP treatment on the basis of conventional treatment can significantly raise the efficacy of treatment in patients with acute left heart failure and respiratory failure.
