小剂量左甲状腺素治疗早产儿暂时性低甲状腺素血症疗效观察

目的观察补充小剂量左甲状腺素对早产儿暂时性低甲状腺素血症的近期疗效。方法将符合条件的68例暂时性低甲状腺素血症早产儿随机分为治疗组34例和对照组34例,两组均给予早产儿常规治疗,治疗组在此基础上给予小剂量左甲状腺素口服。观察两组在体重增长、喂养不耐受持续时间、静脉高营养时间、住院时间、黄疸持续时间及新生儿神经行为测定(NBNA)评分等方面差异。结果观察组静脉营养、黄疸持续时间、喂养不耐受持续时间及住院时间比对照组明显缩短(P0.05),NBNA评分及体重增长方面两组对比无明显差异(P0.05)。结论对低甲状腺素血症早产儿早期给予小剂量补充甲状腺素可以促进胃肠道营养,缩短住院时间,减少家长经济负担。

Clinical observation on the efficacy of ievothyroxine in treatment of low level of thyroid hormone in prematures.

Objective To observe the short - term curative effect of supplementary therapy with levothyroxine tablets in treatment of tran- sient hypothyroxinemia in prematures. Methods Sixty eight prematures with hypothyroxinemia were divided into treatment group （ 34 prema- tures） and control group （ 34 prematures） ; the premature infants in treatment group were treated with oral administration of levothyroxine tablets. The ponderal growth, duration of feeding intolerance, duration for intravenous nutrition, duration of jaundice, hospitalization days and NBNA scores were observed and compared between these two groups. Results The duration for intravenous nutrition, duration of jaundice, duration of feeding intolerance, and hospitalization days in treatment group were statistically shorter than those of control group （ P 〈 0.05 ） . The difference in ponderal growth and NBNA scores between these 2 gxoups was significant （ P 〉 0.05 ） . Conclusion Early supplementation of levothyroxine tablets in treatment of transient hypothyroxinemia in premature infants is safe and effective, and it can promote gastrointestinal nutrition of prema- ture infants and reduce economic burden of their parents.
