肝素治疗小儿重症肺炎的疗效及对血小板参数和D-二聚体的影响

目的探讨肝素治疗小儿重症肺炎的疗效及对血小板数参数和D-二聚体（D—Dimer）的影响。方法将95例小儿肺炎患儿随机分为治疗组48例和对照组47例。两组患儿均给予常规治疗，治疗组在此基础上加用小剂量肝素治疗。于治疗前及治疗后检测两组血小板参数及D—Dimer浓度。结果治疗组的总有效率为95．8％，显著高于对照的76．6％（P〈0．01）。经治疗后，治疗组的血小板参数测定值较治疗前明显改善（P〈0．01），且与对照组比较有极显著性差异（P〈0．01）；治疗组D—Dimer升高者仅占6．3％，显著低于对照组的51．1％（P〈0．01）。结论肝素治疗小儿重症肺炎疗效显著，能明显改善患儿的高凝状态。

Curative Effect of Heparin in the Treatment of Children with Severe Pneumonia and Its Influence on Platelet Parameters and D-Dimer

Objective To observe the therapeutic effects of heparin in the treatment of children with severe pneumonia and its influence on platelet parameters and D-Dimer. Methods Ninety-five children with severe pneumonia were divided randomly into treatment group （n=48） and control group （n=47）, two groups received conventional treatment, moreover, the treatment group was treated with subcutaneously small dosage of heparin. Serum platelet parameters and D-Dimer were measured before and after treatment. Results The total effectiveness rate of the treatment group was significantly higher than that of control group （95.8% vs 76.6%, P 〈0.01）. Comparison with those before treatment, the platelet parameters in the treatment group improved obviously after treatment （P 〈0.01）, and there was significant difference as compared with control group （P〈0.01 ）. The rate of increased D-Dimer in therapy group was significantly lower than that of control group after treatment （6.3% vs 51.1%, P 〈0.01）. Conclusion Heparin is an effective drug for the treatment of children with severe pneumonia, which can obviously improve blood hypercoagulable state.
