血清肌钙蛋白I与慢性心力衰竭患者室性心律失常的关系

摘    要:
目的探讨慢性心力衰竭(chronic heart failure,CHF)患者血清心肌肌钙蛋白I(cardiac troponin I,cTnI)与室性心律失常的关系。方法根据入院时血清cTnI检测结果将218例CHF患者分为cTnI阳性组(cTnI≥0.05ng/mL)108例和cTnI阴性组(cTnI〈0.05ng/mL)110例。2组患者均行24h动态心电图监测。结果cTnI阳性组平均每小时单个室性异位搏动的个数、平均每小时成对室性异位搏动的个数、24h室性心动过速的次数均明显高于cTnI阴性组(P均〈0.05)。多变量logistic回归分析结果显示,cTnI阳性是CHF患者室性心律失常的预测因子。结论cTnI阳性与CHF患者室性心律失常密切相关。

Abstract:
Objective To elucidate the relationships between serum cardiac troponin I(cTnI) and ventricular arrhythmia in patients with chronic heart failure(CHF). Methods Two hundred and eighteen patients with CHF were divided into two groups: cTnI-positive group with serum cTnI≥0.05 ng/mL(n= 108) and cTnI-negative group with serum cTnI〈0.05 ng/mL(n= 110). The assessed by 24 h Holter monitoring in two groups. Results The cTnI-positive group number of mean hourly ventricular pairs, mean hourly number of single ventricular ectopic beats and the frequency of ventricular tachycardia episodes per 24 h were higher in cTnI-negative group(all P〈0.05). In multivariate logistic regression, cTnI-positive could be a predictor of ventricular arrhythmias in CHF patients. Conclusion cTnI-positive closely related with ventricular arrhythmia in CHF patients.
