我院输注新鲜冷冻血浆的合理性及有效性分析

目的探讨FFP在临床中应用的效果及临床科学、合理应用FFP的情况。方法收集2007年1月。2008年4月我院输注新鲜冷冻血浆（FFP）共200例（516次）的资料，分析临床医生输注FFP的目的；输注FFP的516例中有256例在输注前以及输注后8h内均检查了PT、APTT，根据现代FFP输注适应证标准，将256例分为非控制组56例和控制组200例，比较输注前及输注后8h内的Pr、APTT的检查结果并进行统计学分析。结果200例中临床医生输注FFP合理使用119例（59．5％），输注血浆1027U（72．4％），不合理使用81例（40．5％），输注血浆391U（27．6％）；非控制组在输注FFP后的PT、APTT的值较输注前有缩短，但差异无统计学意义（P〉0．05）；控制组在输注FFP后PT、APTT的值较输注前有明显缩短，差异有统计学意义（P〈0．05）。结论根据现代FFP输注适应证标准，合理使用FFP能有效纠正凝血因子缺乏所致的出血；科学、合理、有效使用新鲜冷冻血浆的意识仍待加强。

Analysing on reasonable and effective which once trannsfused FFP in our own hospital

Objective Aiming at discussing the effect of FFP transfusion and the present state of scientific and reasonable FFP transfusion in clinic. Methods Collecting 200 patients＇ （516 cases）date, which once trannsfused FFP in our own hospital from Jan. 2007 to Apr.2008,then analysing the purpose of FFP transfusion. In these 516 cases,there are 256 cases which tested PT＆APTT both before and after FFP transfusion in 8 hours. According to the modem FFP transfusion standard, we separated the 256 into two groups-the uncontrolled group（56 cases ）and the controlled group （200 cases）, than compared the pre-transfusing PT＆APTT and after-transfusing ones and analysed these test results statistically. Results There were 119 patients whose FFP transfusion were reasonale in the total 200 patients,took 59.5% ,plasam 1027U,took 72.4% ;81 patiants were unreasonable,took 40.5%, plasam 391U, took 27.6%. Although the after-transusing PT＆APTT were shortened than the pre-transfusing ones in the uncontrolled group, there had no significance in statistic （P 〉 0.05 ）. Meanwhile in the controlled group the after-transfusing PT＆APTT were largely shortened than the pre-transfusing ones, which had significance in statistic （ P 〈 0.05 ）. Conclusion According to the modem FFP transfusions tandard, to use FFP reasonably can inpove the blood coagulation function and connecting the bleeding. The ideology of how to use FFP scientifically, reasonably and effectively has large space to strengthen.
