摘    要:目的探讨应用术中透视和同轴套管通道对骨骼病变进行微创活检的临床价值。方法对33例患者,其中四肢骨29例,髂骨4例,借助术中透视机定位,并用同轴套管软组织扩张系统通过皮肤小切口建立工作通道,钻开病变处骨质,髓核钳夹取病变内成形病变组织送检。结果33例患者都成功取出病变组织,病理检查均明确诊断,无一例严重并发症。结论术中透视同轴套管通道微创活检术可获得类似开放活检获取成形病变组织块的优点,活检阳性率和病检确诊率高,且患者创伤小,恢复快,并发症少,是一种经济简便,安全有效的诊断方法,值得进一步临床推广。

Abstract:Objective To evaluate the clinical value of the minimally invasive biopsy of bone lesion with the help of image intensifier and co-axial soft tissue expander. Methods Thirty-three patients with bone lesions which were 29 lesions in limbs and 4 lesions in iliac, underwent minimally invasive biopsy. Lesions were first located with image intensifier, then a working channel was established with co-axial soft tissue dilator through a small skin incision. A small hole was drilled through the cortex over the lesion, and the lesion tissue was taken out with rongeur. Results Lesion tissues were obtained from all 33 patients and confirmative pathological diagnosis were given to every patient. The rate of diagnosis was 100%. No serious complication occured. Conclusions Minimally invasive biopsy of bone lesion with image intensifier and co-axial soft tissue dilator can get target lesion tissue easily, and the rate of pathological diagnosis is high, while the patient gets less surgical invasion, and recovers more quickly, with little complication. It is a safe and cheap biopsy method and deserves furthur clinical rsearch and application.
