直肠癌术前放疗和单纯手术疗效比较分析

目的探讨直肠癌术前放疗是否减少局部复发率并延长生存率。方法全部病例采用Mile’s和Dixon术式，并随机分为术前放疗组32例和单纯手术组32例；术前放疗组设盆腔后野和两侧野照射，照射总剂量45Gy，每周5次，每次3Gy，放疗结束后2-3周进行手术。结果术前放疗组32例1／2、1、2年的局部复发率为3．13％、9．38％、18．75％；单纯手术组25例为6．25％、28．13％、37．50％，术前放疗组1、2年的病人生存率为93．7％、87．5％；单纯手术组为46．88％、43．75％、两组1、2年局部复发率及生存率均有显著差异。结论直肠癌手术前放疗组局部复发率显著低于单纯手术组，且术后生存率显著高于单纯手术组。

Comparion of the preoperative radiotherapy of rectal carcinoma and with only operation

Objective To study the influence of the preoperative radiotherapy(PROT)on the local recur-rent rate and prolong existence rate of rectal carcinoma.Methods All patients were treated by Mile’s or Dixon operations ;and random-ized into two groups before operation,PORT group(32 cases) were given radiotherapy to a total dose of 45Gy in posterior field and both side field,5 times every week,each time 3Gy,the other group(32 cases) were treated with only operation.Results The 1/2、1-and 2 years local recurrent rates were 6.25%,28.13%,37.50%in only operation group;and 3.13%,9.38%,18.75%in PROT group.The 1-and 2 year existence rate were 93.7%,87.5% in PROT group ;and 46.88%,43.75% in only operation group.Spectively The dif-ferences of local recurrent rates were significant between both groups after 1 year,and after 2 years.Conclusion The local recurrent rate of PROT group was obviously decreased in comparison with the only operation group,and the existence rate is higher than the only operation group.
