足月胎膜早破分娩方式不同对围生儿的影响

目的了解足月胎膜早破分娩方式不同对围生儿的影响。方法选择350例足月单胎头位住院分娩孕妇,胎膜早破者150例为研究组（早破组）,无胎膜早破者200例为对照组,分析对比两组胎儿娩出方式的差异及不同分娩方式围生儿结局的差异。结果早破组阴道分娩率低于对照组,差异有统计学意义,剖宫产率高于对照组,差异有统计学意义。新生儿窒息率及吸入性肺炎率两组内剖宫产与自然分娩比较差异均无统计学意义,早破组中缩宫素引产窒息率较自然分娩发生率高,差异有统计学意义。新生儿高胆红素血症发生率两组内缩宫素引产及剖宫产较自然分娩升高,差异有统计学意义。结论足月胎膜早破一旦发生应尽快终止妊娠,破膜〉12 h积极给予广谱抗生素预防感染。加强围生儿护理,严密监测,积极喂养,改善预后。

Influence of different parturition pattern after the normal period of gestation with premature rupture of membranes on the perinatal infant health

Objective To investigate the influence of diffent parturition pattern after the normal period of gestation with premature rupture of membranes on the perinatal infant health.Methods Three hundred and fifty cases of the full term pregnant women with singleton and cephalic presentation were examined during hospitalization.Among of two groups,150 cases in group of premature rupture of membranes,and 200 cases in group without premature rupture of membranes.Analyse and compare the influence to the perinatal infant health of two groups with parturition pattern with born after the normal period of gestation.Results The incidence of spontaneous vaginal delivery was lower and cesarean rate was higher in the group with premature rupture of membranes comparing with the group without premature rupture of membranes.And the rate of asphyxia and aspiration pneumonia was without significant difference between the two groups.The rate of induced labour with oxytocin was higher than the matched group.It was higher incidence of hyperbilirubinemia of neonatus and induced labour with oxytocin and cesarean section compare with the spontaneous labor group.Conclusion It is essential with born after the normal period of gestation to terminate pregnancy in time when premature rupture of membranes appear.
