可吸收内固定材料治疗髌骨骨折的临床研究

摘    要:
目的探讨可吸收内固定材料在髌骨骨折治疗中的临床疗效及优缺点。方法76例新鲜髌骨骨折的患者,手术采用膝前纵行直切口,复位骨折端,然后选用可吸收螺钉固定。再用可吸收缝线修复髌前筋膜以及股四头肌扩张部。术后均用石膏托外固定膝关节4—6周,拆除石膏托后进行膝关节功能锻炼。结果有72例获得随访,随访时间6~24个月,骨折全部愈合,骨折愈合时间6—12周,膝关节功能屈伸恢复理想,优良率95.8%。结论可吸收螺钉联合可吸收缝线治疗髌骨骨折临床疗效理想,无需二次手术取出内固定,减少了再次手术给患者带来的痛苦及风险,该方法值得临床应用、推广。

Abstract:
Objective To explore the clinical effects, the advantages and disadvantages of the treatment of patella fracture with absorbable screws and absorbable suture. Methods 76 cases of fresh patellar fracture were treated with absorbable internal fixation devices. The prepatellar longitudinal incision was used, the fractured bone was restored and fixed by two absorbable screws. The absorbable suture was used to repair the prepatellar fascia and quadriceps expansion unit. The plaster immobilization was applied for 4 -6 weeks after operation, and the knee joint training was carried out after removing the plaster immobilization. Results 72 cases were followed up for 6 - 24 months, and all the fractures were healed ; the healing time was 6 - 12 weeks. The flexion-extension function of the knee recovered well, and the excellent and good rate was 95.8%. Conclusion The absorbable screw combining absorbable suture is an ideal clinical treatment for patellar fracture, and the fixation devices do not need to be removed, which reduces the pain and risks of the reoperation. The method is worthy of clinical application and recommendation.
