万拉法新与丙米嗪治疗脑梗塞后抑郁障碍对照研究

目的　探讨国产万拉法新胶囊在治疗脑梗塞后抑郁障碍的疗效 ,不良反应及其安全性。方法　通过随机双盲对照研究 ,共纳入万拉法新组 3 6例 ,丙米嗪组 3 6例 ,用 HAMD,CGI和 Tess量表评定疗效和不良反应。结果　国产万拉法新胶囊对脑梗塞后抑郁障碍治疗有效 ,有效率在 88.88% ,与丙米嗪组 86.1 1 %相当 ;其量表评定有效率在 86.1 1 % ,与丙米嗪组83 .3 3 %亦无显著性差异 ( P<0 .0 5 ) ;在不良反应方面 ,万拉法新与丙米嗪相比 ,万拉法新在治疗后 Tess植物神经系统、心血管系统及其它系统因子分布明显降低 ,差异性具有显著性 ( P<0 .0 0 1 ) ,而 Tess化验异常 ,行为毒性因子两组相比无明显差异 ( P>0 .0 5 )。结论　万拉法新治疗脑梗塞后抑郁障碍疗效肯定 ,不良反应少于丙米嗪 ,在临床上值得推广应用。

Comparison of Venlafaxine with Imipramine on Post-stroke Depression

Objective To study the therapeutic effect,side effect and security of domestic venlafaxine on post-stroke depression.Methods 72 patients of post-stroke depression are blindly divided into two groups in random,36 cases are taken as imipramine therapeutic group,another as venlafaxine therapeutic group.Their therapeutic effect and side effect are blindly measured with HAMD,CGI,Tess respectively.Results The effective rate in venlafaxine group is 88.88%,86.11% in imipramine group,there were not difference significant(P>0.05);when it is valued with scales,86.11% in venlafaxine group and 83.33 % in imipramine group there were not difference significant(P>0.05).When their side effect is concerned,Tess factor scores of vegetative nervous system ,heart-vessel system in venlafaxine group is significantly lower than imipramine group(P<0.01);but Tess test abnormal factor scores and behaviour toxical factor scores are not significant(P>0.05).Conclusion The therapeutic effect of venlafaxine for post-stroke depression is sure,and side effect is little.It is deserved to enlarge application of venlafaxine.
