| 摘 要： | 目的:探讨老年尿路感染患者多重耐药菌(MDR)分布状况、细菌耐药特性及其相应护理措施。方法:对从73例老年尿路感染患者中分离的76株MDR研究分布特征、耐药特性并给予针对性护理。结果:76株MDR中革兰阴性杆菌占78.9%,革兰阳性菌占21.1%,所有细菌均有较强耐药性,其中革兰阴性杆菌MDR对亚胺培南西司他丁钠(IMP/CS)、头孢哌酮/舒巴坦(CFP/SU)、哌拉西林/他唑巴坦(PIP/TZ)耐药率最低,但部分药物出现耐药或中介;嗜麦芽寡氧菌对IMP/CS完全耐药,鲍曼不动杆菌、铜绿假单胞菌对IMP/CS耐药率达到33.3%、38.5%;革兰阳性球菌MDR主要以凝固酶阳性葡萄球菌和肠球菌为主,对万古霉素完全敏感,对利福平、氯霉素耐药性较低。结论:老年患者尿路感染MDR以革兰阴性杆菌为主,所有MDR耐药性均较强,仅对少数抗生素敏感。须重视MDR感染问题,加强针对性护理,以确保患者顺利康复。 |
