血管腔内重建在髂动脉病变引起的难治性动脉性勃起功能障碍治疗中的应用

目的评估血管腔内重建治疗因路动脉病变引起的动脉性勃起功能障碍的疗效及安全性。方法选取2017年4月至2019年1月在本院收治的经动脉造影确诊的动脉性勃起功能障碍患者8例,进行术前评估后行髂内动脉或髂总动脉腔内血运重建术,术后3个月内口服抗血小板药物.随访时间6个月。结果8例患者手术顺利,术中未出现特殊并发症,其中2例患者置人髂总动脉支架,2例置入髂内动脉支架,患者手术后阴茎肱动脉压力指数、勃起功能评分均高于术前,差异有统计学意义(P<0.01),而术后6个月勃起功能评分较术后3个月时有所下降,差异有统计学意义(P=0.015)。结论血管腔内重建治疗可以改善阴茎动脉血供,从病因学上解决动脉性勃起功能障碍问题,随访期内未有特殊并发症及出血事件报道,具有良好的疗效及安全性。

Application of intravascular reconstruction in the treatment of refractory arterial erectile dysfunction caused by iliac artery disease

Objective To evaluale the efficacy and safety of endovaseular reconstruetion in the treatment of arterial erectile dysfunetion.Methods Eight patients with arterial erectile dysfunction were diagnosed by angiography were enrolled from April 2017 to January 2019,and preoperative as-sessments were performed.After locate the lesion by contrast,the intermal ilac artery or the common ilac artery revascularization was performed.Antiplatelet drugs were given within 3 months after surger-y,and the follow-up period was 6 months.Results Eight patients underwent successful operations without special complications.Among all the 8 cases,two patients were implanted with common iliac artery stent.Two were implanted with internal iliac artery stent while others were treated with balloon dilation.The statistics ollected reported significant dffrerence between preoperative data and postoper-ative data of PBI and erectile function scores(P<0.01).The improvement of erectile function scores maintained in 6 months(P<0.05),while the score tend to go down slowly over time.Conclusions Endovascular treatment can improve the blood supply of the penile artery and solve the problem of arte-rial erectile dysfunetion from the perspective of etiology.There are no special complications and bleed-ing events during the fllow_up period.However,it requires futher exploration to confirm the longl term efficacy and safety.How to maintain the long-term patency rates to prevent restenosis is another important project to study.
