腰硬联合麻醉在老年妇科肿瘤手术中的应用

目的：研究腰-硬联合麻醉（CSEA）在老年妇科肿瘤手术中的应用价值。方法：将130例择期手术的老年妇科肿瘤患者随机均分为CSEA和EA组,CSEA组行硬腰联合麻醉,EA组行单纯硬膜外麻醉。观察两组麻醉后的麻醉起效时间、阻滞完全时间、局麻药用量、肌松满意程度、无痛率以及不良反应。结果：CSEA组麻醉诱导时间及阻滞完全时间明显短于EA组,硬膜外局麻药用量明显少于EA组（P〈0.01）。除低血压外,两组不良反应比差异无显著性（P〉0.05）。结论：腰硬联合麻醉应用于老年性妇科肿瘤手术,是一种简易可靠易行的麻醉方法,具有起效快,维持时间长,局麻药用量少,不良反应发生率低,是一种值得推荐的麻醉方式。

The Application of Spinal-epidural Anesthesia in Elderly Patients with Gynecological Neoplasm's Surgery

Objective： To study the value of spinal-epidural anesthesia in elderly patients with gynecological neoplasm’s surgery.Method： Elective 130 cases of elderly patients with gynecological neoplasm’s surgery were randomly divided into combined spinal and epidural anesthesia group（CSEA group）and epidural anesthesia group（EA group）,65 cases in each group.Onset of anesthesia were observed,perfect time for local anesthetic dosage,muscle satisfaction,painless rate and adverse reaction.Result： CSEA group of local anesthetic dosage was less than EA group,sensory block onset time,improved the time of arrest was significantly shorter in the EA group（P〈O.01）.Besides hypotension,adverse reaction in the two groups had no significant difference（P〉0.05）.Conclusion： Combined spinal and epidural anesthesia for gynecological neoplasm’s surgery in elderly patients is a reliable and feasible anesthesia,which has fast efficiency,long maintenance period,less local anesthetic dosage and low adverse reaction.It is an anesthetic approach to recommend.
