摘    要:目的 建立肺结核患者给予利福平后血药浓度的监测方法,为临床合理用药提供参考.方法 高效液相法,色谱柱为安捷伦C18柱,(5μm,4.6×250 mm);流动相为甲醇:乙腈:0.04 mol×L-1 KH2PO3( 30:30:40);流速1.2 ml/min,检测波长为340 nm;柱温20℃.结果 血浆中利福平在0.30 ~24 μg/ml范围内浓度与峰面积线性关系良好,r=0.9995,平均回收率为98.87%.140例肺结核患者静滴或口服利福平450 mg后,其中126例血药浓度检测结果在0.50~12.75 μg/ml之间,12例血药浓度检测结果0.30~0.5 μg/ml之间,2例血药浓度小于0.3μg/ml.结论 该方法准确、灵敏,专属性强,重现性好,适用于治疗药物监测,可为临床合理用药提供数据参考.

Abstract:Objective To monitor plasma rifampicin concentrations in patients with pulmonary tuberculosis by HPLC.Methods The assay was conducted on C18 GL sciences column(5μm,4.6×250 mm)with the mobile phase of the mixture of CH3OH-CH3CN-0.04 mol/L KH2PO3(30:30:40)at a rate of 1.2 ml/min,the UV detection wavelength was 340 nm.The column temperature was 20℃. Results A good linearity was obtained from 0.30~24μg/ml with a correlation coefficient of 0.999 5.The average recovery of the assay was 98.87%.Among 140 patients administrated with 450 mg rifampicin orally or intravenously,the plasma drug concentration ranged 0.50~12.75μg/ml in 126 cases,ranged 0.30~0.5μg/ml in 12 cases and was less than 0.3μg/ml in 2 cases.Conclusion The method applied in this study is accurate and sensitive,which can be used to monitor plasma rifampicin concentration for rational drug administration in clinical practice.
