摘    要:
目的分析早期强直性脊柱炎(AS)患者X线、CT和MR影像学特征,并比较这3种影像检查方法在诊断早期AS中的作用。方法对46例临床诊断为AS早期(0~Ⅱ级)患者分别行X线、CT和MR检查。分别评价这3种检查方法在0~Ⅱ级AS病变中的检出率,并对检查结果行统计学分析。结果对于Ⅰ~Ⅱ级病变,MR(36/38)较CT(27/46)发现更多病变,两者诊断符合率分别为94.7%和58.7%,差异有统计学意义(P<0.01),但对于Ⅰ、Ⅱ级病变的区分,MR与X线、CT比较差异无统计学意义(P>0.05)。结论早期AS的影像学检查中,X线、CT和MR各有特点,具有一定的特异性,MR较X线、CT能发现更早期的病变,影像学的综合检查对病变的早期确诊具有重要意义。

Abstract:
Objective To analyze the imaging features of X-ray,CT and MRI in early ankylosing spondylitis(AS) and to compare the value of X-ray,CT and MRI in early diagnosis of AS.Methods 46 patients clinically diagnosed as AS received X-ray,CT and MRI examinations.Diagnostic rates of the three different imaging methods in 0~Ⅱ degree AS were evaluated and statistical data were also analyzed.Results MRI(36/38) was superior to CT(27/46) in detecting abnormal changes of early AS(Ⅰ~Ⅱ degree),and the diagnostic accordance rate of MRI and CT was 94.7% and 58.7% respectively,the difference was statistically significant(P<0.01).While the difference was not statistically significant between MRI,X-ray and CT in distinguishing Ⅰ and Ⅱ degree AS(P>0.05).Conclusion Different imaging methods(X-ray,CT,MRI) show related imaging features in early AS diagnosis.MRI is superior to X-ray and CT in the early detecting of AS abnormal changes.A combination of imaging methods is important in early AS diagnosis.
