全子宫切除术后对卵巢血流及卵巢功能的影响

目的探讨全子宫切除术后对卵巢血流及卵巢功能的影响。方法年龄≤46岁的未绝经子宫肌瘤患者69例,分成2组：A组,共38例,〈40岁;B组,共31例≥40岁。分别在术前、术后3月、术后6月观察卵巢血流的变化,测定血清中卵泡刺激素（FSH）、黄体生成素（LH）、雌二醇（E2）、孕酮（P）观察卵巢功能的变化。结果两组治疗后3月、6月的卵巢血流RI及PI值明显较术前增高（P〉0.05）。两组患者术前FSH、LH、E2、P均在同一水平（P〈0.05）;术后6月B组的E2水平明显低于术前水平（P〈0.05）。结论全子宫切除术可影响术后患者血流及卵巢的内分泌功能,在临床中应加强术后随访。

The effect of complete hysterectomy on ovarian hemodynamics and ovarian function

Objective To investigate the effect of complete hysterectomy on ovarian hemodynamics and ovarian function.Methods 69 patients of ≤46 y were divided into 2 groups：group A（n=38〈40 y）and group B（n=31,≥40 y）.The changes of ovarian hemodynamics（velocity maximum,MAX;velocity minimum,MIN;time velocity maximum,TMAX;pulsatility index,PI）resistance index,RI and ovarian function（LH,E2 and P）were measured before operation,3 month and 6 month after operation.Results The RI and PI of 2 group were increased significantly（ P 〈 0. 05 ）. The levels of E2 in group B of 6 month after operation were decreased significantly（ P 〈 0. 05 ）. Conclusion complete hysterectomy can influence the ovarian hemodynamics and ovarian function and follow-up visit should be enhanced to the patients of poastoperation of complete hysterectomy.
