延续护理在心脏瓣膜置换术后患者中的应用

目的 探讨延续护理在心脏瓣膜置换术后患者抗凝治疗过程中的应用效果.方法 选取我科2012年1月~2012年10月行心脏瓣膜置换术的患者80例,按照随机数字表法分成试验组和对照组各40例.对照组在住院期间和出院后给予常规的治疗、护理、健康教育及出院指导,试验组在此基础上出院后给予系统的延续护理服务,分别于出院后3、6个月对两组患者进行抗凝相关知识、抗凝药物服用的依从性调查.结果 出院后3、6个月试验组抗凝相关知识的掌握程度、对抗凝治疗的依从性明显高于对照组,差异均有统计学意义（P〈0.05）.结论 心脏瓣膜置换术后抗凝治疗是一个长期的过程,延续护理服务能有效提高患者服药的依从性,使抗凝治疗更安全、有效.

The application of transition nursing in patients underwent heart valve replacement surgery

Objective To explore the application of transitional nursing in anticoagulant therapy of patients underwent heart valve replacement surgery （HVRS）. Methods 80 patients underwent HVRS were randomized to experimental （40 cases） and control group （40 cases）. Regular nursing and treatment were given to both groups in hospital. However, the patients of exper- imental group accepted transitional nursing out of hospital, whereas the common discharge guidance was given to control group. All the patients were surveyed ol~anticoagulant knowledge and medication adherence at 3 rd and 8 th month out of hospital. Results Ex- perimental group performed far better than control group in the surveys. Conclusion Transitional nursing can significantly improve the medication adherence of patients underwent HVRS, which making anticoagulant therapy more efficient and safe.
