二次剖宫产产妇术后出血影响因素分析与护理对策

目的探讨二次剖宫产产妇术后出血的影响因素与护理对策。方法选取2017年1月至2019年1月我院妇产科收治的60例二次剖宫产产妇为研究对象,分析二次剖宫产产妇产后出血的影响因素,并制定相应护理对策。结果单因素分析显示,产妇的分娩年龄、妊娠合并症和并发症、多胎妊娠、胎盘因素、缩宫素联合米索前列醇使用、手术时机选择和初次剖宫产产后出血都是二次剖宫产产妇术后出血的影响因素。多因素logistic回归分析显示,缩宫素联合米索前列醇使用和初次剖宫产产后出血是二次剖宫产产妇术后出血的独立影响因素。结论二次剖宫产产妇出血率较多,医护人员需加强二次剖宫产产前高危因素筛选和管理,减少社会因素剖宫产例数,采取科学护理对策降低二次剖宫产产后出血。

Analysis of influencing factors of postoperative hemorrhage in second-time cesarean section and its nursing countermeasures

Objective To explore the analysis of influencing factors of postoperative hemorrhage in second-time cesarean section and its nursing countermeasures.Methods To select 60 cases of maternity patients undergoing second-time cesarean section who were admitted to the obstetrics and gynecology department of our hospital from January 2017 to January 2019 as the study objects.The influencing factors of postpartum hemorrhage of maternity patients undergoing second-time cesarean section were analyzed to formulate corresponding nursing strategies.Results Univariate analysis showed that the maternity patients’delivery age,pregnancy complications,multiple pregnancies,placental factors,use of oxytocin combined with misoprostol,selection of timing of surgery and postpartum hemorrhage after the first cesarean section were all the influencing factors of postoperative hemorrhage in second-time cesarean section.Multivariate logistic regression analysis showed that the use of oxytocin combined with misoprostol and postpartum hemorrhage after the first cesarean section were independent influencing factors of postoperative hemorrhage in second-time cesarean section.Conclusion The second-time cesarean section had a more bleeding rate.Medical staff should strengthen the screening and management of high-risk factors before second-time cesarean section,reduce the number of cases of cesarean section due to social factors,and take scientific nursing measures to reduce the postoperative hemorrhage in second-time cesarean section.
