等速肌力训练对脑梗死偏瘫功能恢复的影响及左右步幅差、步速与机体平衡能力、步行能力相关性分析

摘    要:目的分析等速肌力训练对脑梗死偏瘫患者疗效评定效果,探讨其下肢功能间的相关性。方法选取我院2017年7月至2019年7月收治的90例脑梗死偏瘫患者为研究对象,随机将其均分为研究组和对照组,对照组实施常规康复训练,研究组实施股四头肌和腘绳肌肌力训练,对比两组相同训练时间日常生活活动能力(Barthel)评分和下肢运动功能评分(Fugl-Meyer)的下肢功能评分、疗效与下肢功能的相关性。结果研究组Barthel和Fugl-Meyer的下肢功能评分显著高于对照组,差异有统计学意义(P<0.05);研究组步态对称性、步速、平衡功能及步行能力改善效果均显著优于对照组,差异有统计学意义(P<0.05);左右步幅差、步速、平衡功能及步行能力之间均存在显著相关性(P<0.01)。结论脑梗死偏瘫应用等速肌力训练,可有效增强患肢膝屈伸肌群肌力,提高步态对称性、步速、步行能力及平衡功能恢复,促进回归正常生活和工作。

Abstract:Objective To analyze the effect of isokinetic strength training for the evaluation of curative effect of patients with cerebral infarction and hemiplegia,and to explore the correlation between lower limb functions.Methods We selected 90 cases of patients with cerebral infarction and hemiplegia who were treated in our hospital from July 2017 to July 2019 as the study objects.They were randomly divided into a study group and a control group.The control group received routine rehabilitation training,and the study group received the strength training of quadriceps femoris and hamstring muscles to compare the correlation between activities of daily living(Barthel)scores and lower extremity motor function score(Fugl-Meyer)of lower extremity function scores,curative effect and lower extremity function in the two groups at the same training time.Results The lower extremity function scores of Barthel and Fugl-Meyer in the study group were significantly higher than those in the control group,and the differences were statistically significant(P<0.05);the improvement effects of gait symmetry,walking speed,balanced function,and walking ability of the study group were significantly better than those of the control group,and the differences were statistically significant(P<0.05);there were significant correlations between left and right stride differences,walking speed,balanced function,and walking ability(P<0.01).Conclusion The application of isokinetic strength training for patients with cerebral infarction and hemiplegia could effectively strengthen the flexor and extensor muscles of the affected limb,improve gait symmetry,walking speed,walking ability and recovery of balanced function,and promote return to normal life and work.
