基于Delphi法构建全膝关节置换快速康复护理模型

目的以“知-信-行”模式为框架构建快速康复外科理念下全膝关节置换康复护理模式。方法以“知-信-行”模式为理论基础,采用文献研究及半结构式访谈以及小组讨论构建初级快速康复外科全膝关节置换康复护理模型;运用Delphi法对20名专家进行2轮函询调查,修改与完善初级模型各级指标。结果2轮函询问卷的有效回收率均为100%;专家权威系数为0.88~0.96,平均权威系数为0.92;专家咨询意见协调度肯德尔和谐系数(W)为0.53;最终确立的康复护理模式包含一级指标3个,二级指标12个,三级指标49个。结论所构建的快速康复外科全膝关节置换康复护理模式结果可靠,可供全膝关节置换患者护理参考。

Construction of a rapid rehabilitation nursing model of total knee replacement based on Delphi method

Objective To construct a rapid rehabilitation nursing model of total knee replacement under the concept of fast-track surgery through the"knowledge-attitude-practice(KAP)"model as a frame.Methods Based on the"KAP"model as the theoretical basis,literature research and semi-structured interviews,and group discussions were used to construct a rehabilitation model of primary rapid rehabilitation surgery with total knee replacement.Delphi method was used to conduct 2 rounds of consultation with 20 experts investigate,revise and improve the indicators of the primary model at all levels.Results The effective recovery rate of the two rounds of questionnaires was 100%,the expert authority coefficient was 0.88-0.96,and the average authority coefficient was 0.92,the Kendall’s harmony coefficient of the coordination degree of expert consultation opinions(W)was 0.53,and there were 3 first grade indicators,12 second grade indicators and 49 third grade indicators in rehabilitation nursing model.Conclusion The results of the fast-track surgery of total knee replacement rehabilitation nursing model constructed were reliable,and it can be used as a reference for the nursing of patients with total knee replacement.
