神经外科重症监护病房气管切开患者肺部感染的目标性监测和干预

目的通过开展目标性监测，针对影响因素采取干预措施，探讨降低气管切开患者肺部感染发生率的有效方法。方法对我院颅脑外科重症监护病房内2009年1—4月气管切开患者肺部感染病例进行回顾性调查，分析了可能引起的因素并采取了相应的干预措施；并于2009年5月起进行目标性监测和干预，对气管切开患者肺部感染率进行前瞻性调查。结果2009年1—14月气管切开患者肺部感染发生率为33．33％；2009年5月至2010年5月感染率为16．33％，两组比较差异有统计学意义（x2=4．352，P〈0．05）。结论开展目标性监测、建立监测系统、规范操作，并开展系列教育项目和现场干预，可以明显降低气管切开患者肺部感染的发生率。

Targeted surveillance and interventions on lung infection of tracheotomy patients in neurosurgery ICU

Objective To explore an efficient way to reduce the incidence of lung infection ot tracheotomy patients by taking targeted surveillance and interventions.Methods Lung infection among tracheotomy patients in neurosurgery ICU of our hospital from January 2009 to April 2009 was retrospectively investigated.Related factors were analyzed and targeted interventions were taken.From May 2009,targeted surveillance and interventions was implemented and incidence of lung infection of tracheotomy patients was perspectively investigated.Results Incidence of lung infection of tracheotomy patients from January 2009 to April 2009 was 33.33％ and that from May 2009 to May 2010 was 16.33％.There was significant difference between the two groups ( x2 =4.352,P ＜ 0.05 ).Conclusions Incidence of lung infection of tracheotomy patients can be effectively decreased by implementing targeted surveillance,constructing surveillance system and standard operation,as well as extending series education project and scene intervention.
