小肠移植的围手术期营养支持治疗（附两例报告）

目的 通过对两例临床小肠移植围手术期营养支持疗效的总结，探讨小肠移植中合理的围手术期营养支持治疗方案。方法 对两例小肠移植患者进行围手术期的营养支持治疗，术前采用PN辅助EN的营养支持方案改善患者营养状况，术后采用先给予TPN，接着PN+EN，逐渐过渡到TEN的营养支持方案。观察患者术前营养状况的改善及术后小肠吸收功能恢复情况及患者体重及各项指标的变化情况。结果 两例患者术前行营养支持治疗后体重均有所增加，营养状况改善，顺利进行手术；术后两例患者小肠吸收功能恢复良好，均从TPN顺利过渡到TEN，时间分别是术后20天和24天。结论 合理的围手术期营养支持治疗是小肠移植成功的关键因素之一。

Perioperative nutritional support of isolated small bowel transplantation

Objective To approach a suitable peri-operative nutrition model from two intestinal transplants. Methods On two cases of small bowel transplantation patients with perioperative nutritional support, preoperative EN with PN auxiliary support improved nutrition status.Postoperative TPN given firstly, then PN+EN, gradual transition to nutritional TEN support .We observed the nutritional status of two patients before transplant and the improvement of intestinal absorption functional recovery after transplants with body weight and changes in the indicators Results All patients of nutritional support had preoperative body weight increase, nutritional status improved after treatment and received operation subsequently. They had intestinal absorption functional well recovery, and smoothly transit from TPN to TEN, 0 and 24 postoperative days respectively. Conclusions Rational perioperative nutritional support is important to provide successful treatment for small bowel transplantation.
