葛根素注射液治疗原发性脑出血的探讨

目的 评价葛根素注射液治疗原发性脑出血(ICH)的疗效及安全性.方法 B组160例采用常规疗法,A组159例在B组治疗基础上,于入院后第3d加用葛根素注射液0.4g GS(或NS)100～250ml静滴,1次/d.结果 入院后第14、28d头颅CT复查脑血肿体积及脑血肿加水肿带体积,2组比较有非常显著性差异(P＜0.01).入院后第14、28d及3个月后CSS和ADL、BI分值评定,2组对比差异有统计学意义(P＜0.01).随着时间延长2组脑心综合征发生率均呈下降趋势,但A组较B组下降幅度大,差异有统计学意义(P＜0.05或P＜0.01).临床疗效评定经Ridit分析,不管是14d还是28d A组均要好于B组,随着葛根素治疗时间延长其临床疗效越加明显.结论 葛根素注射液治疗ICH患者可加快脑血肿吸收,减轻脑血肿周围组织水肿和脑水肿.改善脑血肿周围组织的局部血流量、微循环和脑血流量.改善和恢复脑血肿周围脑组织缺血半影区的神经细胞功能,降低致残率,提高患者的生活质量.同时葛根素注射液又可防治脑心综合征,且安全性高.

Study of radix puerariae on primary intracerebral hemorrhage

Objective To evaluate the efficacy and safety of radix puerariae on primary intracerebral hemorrhage(PICH). Methods 160 patients with PICH were treated with routine therapeutics in group B which was control group.The others 159 patients were treated with radix puerariae 0.4g added to GS or NS 100 to 250ml ivgtt qd based on the treatment of group B on the third day after inpatient.Results Both the volume of hematoma and edema and the volume of hematoma in group A were smaller than those in group B through reexamining CT on 14th and 28th day,and there was significant difference(P<0.01).Chinese stroke scale,ADL and modified Barthol index in group A were better than those in group B on 14th day,28th day and three months later,and there was very significant difference(P<0.01).With extending of time,the incidence of cerebral-heart syndrome in both groups decreased,but the downward tendency in group A was more than that in group B and there was statistics difference(P<0.05or 0.01).The clinical efficacy evaluating analyzed by ridit in group A was better than group B,with extending of time treated with radix puerariae,the clinical efficacy was more significant. Conclusion Radix puerariae can accelerate the absorb of cerebral hematoma,relieve edema of the cerebral and the edema issue around the hematoma.Improve the local blood-flow,microcirculation and the blood-flow of cerebrium around the the hematoma.Improving and retrieving the function of neuron cell of penumbra region around the hematoma can decrease disabled incidence and increase patient's life quality.Futhermore,it is safe to PICH patients.
