慢性肾病患者血浆leptin、NPY及血清Hcy测定的临床意义

目的：探讨慢性肾病患者血浆leptin、NPY及血清Hcy三项指标水平变化及病情进展的关系。方法：血浆leptin、NPY两项指标均采用放射免疫分析;Hcy则采用化学发光免疫分析。结果：慢性肾盂肾炎组血浆leptin、NPY及血清Hcy三项指标水平与对照组比较略有升高,但均无显著统计差异（P均〉0.05）;糖尿病肾病组血浆leptin和血清Hcy两项指标均显著高于对照组（P均〈0.05）,而血浆NPY水平则与对照组比较略高,但无统计学意义（P〉0.05）;慢性肾衰组血浆NPY水平显著高于对照组（P〈0.05）,血浆leptin和血清Hcy两项指标均非常显著高于对照组（P均〈0.01）。结论：糖尿病肾病组及慢性肾衰组leptin和血清Hcy水平变化较为显著,表明两项指标参与了慢性肾病的病理过程,其测定对于了解病情及评估预后有帮助。

Clinical Significance of Determination of Plasma Leptin, NPY and Serum Hcy Levels in Patients with Chronic Renal Diseases

Objective To study the relationship between progress of disease and blood levels of leptin,NPY,Hcy in patients with chronic renal diseases.Methods Plasma leptin,NPY（with RIA） and serum Hcy（with CLIA） were determined in ① 32 patients with chronic pyelonephritis ② 28 patients with dibetic nephropathy ③ 30 patients with chronic renal failure and ④ 30 controls.Results Blood levels of leptin,NPY and Hcy were slightly higher in patients with chronic pyelonephritis than those in controls but without significance（P0.05）.In patients with diabetic nephropathy,the plasma leptin and serum Hcy levels were significantly higher than those in controls（P0.05）,but the plasma NPY levels were only insignificantly higher（P0.05）.In patients with chronic renal failure,the blood levels of NPY（P0.05） and leptin,Hcy（P0.01） were all significantly higher than those in controls.Conclusion Blood levels of these three parameters especially leptin and Hcy,were increased in patients with chronic renal diseases and the increase was most significant in advanced cases.
