2型糖尿病视网膜病变患者血清脂联素水平的研究

目的 为观察血清脂联素水平的变化与2型糖尿病合并视网膜病变(DR)各期的关系.方法 采用ELISA法对113例2型糖尿病患者血清脂联素水平进行检测,其中2型糖尿病无DR患者30例(DM组)、2型糖尿病合并DR背景型患者45例(BDR组)、2型糖尿病合并DR增殖型患者38例(PDR组)和健康对照30例(NC组).结果 (1)DM组血清脂联素水平与NC组比较差异有统计学意义(P＜0.05);BDR组血清脂联素水平与DM组、NC组比较差异均有统计学意义(P＜0.05或P＜0.01);PDR组血清脂联素水平与BDR组、DM组、NC组比较差异均有统计学意义(P＜0.05或P＜0.01).(2)相关分析发现,2型糖尿病患者血清脂联素水平与收缩压、糖化血红蛋白、TG、TC、LDL-C成显著负相关,而与HDL-C成显著正相关.结论 2型糖尿病患者血清脂联素水平明显降低, 2型糖尿病合并DR后血清脂联素水平更低.脂联素可作为判断2型糖尿病合并DR损害程度的指标.

Investigation on serum level of adiponectin in patients with type 2 diabetic retinopathy

Objective To investigate the change of serum adiponectin level in different stages of diabetic retinopathy(DR).Methods 113 patients with type 2 diabetes mellitus(DM),including 30 patients without DR(DM group),45 patients complicating with background DR(BDR group) and 38 complicating with proliferative DR(PDR group),and 30 healthy controls(served as control group) were enrolled in this study.Enzyme-linked immunosorbent assay(ELISA) was applied to measuring serum level of adiponectin.Results(1) The serum level of adiponectin in DM group was significantly lower than that in control group(P<0.05).There were statistical difference of serum adiponectin level between BDR group and DM group as well as control group(P<0.05 and <0.01 respectively).Serum adiponectin level changed remarkably in PDR group compared with respective one in BDR group,DM group and control group(P<0.05 or <0.01).(2) Correlation analysis revealed serum adiponectin level was closely negative correlation with systolic blood pressure(SBP),glycosylated hemoglobin,triglyeride(TG),total cholesterol(TC),and low density lipoprotein-cholesterol(LDL-C),but significantly positive correlation with high density lipoprotein-cholesterol(HDL-C).Conclusion Serum level of adiponectin in patients with type 2 diabetes mellitus declines significantly,especially for patients complicating with diabetic retinopathy.Adiponectin type may be used as an reliable clinical markers for assessing the severity of DR complicated to type 2 diabetes mellitus.
