抗幽门螺杆菌联合抗抑郁药治疗功能性消化不良疗效观察

目的观察抗幽门螺杆菌联合抗抑郁药、心理疏导辅助治疗伴有焦虑抑郁状态且幽门螺杆菌H.pylori（＋）功能性消化不良（FD）的疗效。方法采用Zung焦虑自评量表（SAS 5.0）和抑郁自评量表（SDS 5.0）筛选出符合罗马Ⅲ诊断标准伴有焦虑抑郁状态的H.pylori（＋）FD患者60例,随即分为A、B两组,A组（n=30）抗幽门螺杆菌联合抗抑郁药黛力新及心理疏导辅助治疗,B组（n=30）仅使用抗幽门螺杆菌治疗,两组患者于停药4周后复查14C-尿素呼气试验以评估H.pylori根除率,比较两组治疗前后症状改善情况、SDS评分。结果 A、B两组临床症状均改善,A组较明显,两组H.p根除率分别为82.5%8,0%,差异无统计学意义（P〉0.05）,A组抑郁量表评分较治疗前提高（P〈0.05）,且与B组比较,差异有统计学意义（P〈0.05）。结论抗幽门螺杆菌联合抗抑郁药、心理疏导辅助治疗伴有焦虑抑郁状态的FD,疗效优于单一使用抗幽门螺杆菌治疗。

The treatment of Anti-Helicobacter pylori combined with Antidepressant medications in patients with functional dyspepsia

Objective To observe The treatment of Anti-Helicobacter pylori combined with Antidepressant medications in patients who had H.pylori（＋） with functional dyspepsia（FD）.Methods 60 patients consistent with anxiety and depression associated with H.pylori（＋）,Zung self-rating anxiety scale（SAS 5.0） and Depression Scale（SDS 5.0） selected the Rome Ⅲ criteria randomly divided into A,B group.A group（n=30） anti-Helicobacter pylori combined antidepressants and psychological counseling deanxit adjuvant therapy,B group（n=30） only the use of anti-Helicobacter pylori treatment,two groups of patients were compared 14C-urea breath test to assess the eradication rate of H.pylori before and after threapy of 4 weeks,meantime compared the improvement of symptoms and SDS score.Results A,B clinical symptoms were both improved,A group was more apparent,the two groups Hp eradication rates were 82.5%,80%,the difference was not statistically significant（P0.05）,The depression scale of A group was improved（P0.05）,the difference was statistically significant with B group（P0.05）.Conclusion The combined anti-Helicobacter pylori anti-depressants,psychotherapy adjuvant treatment of anxiety and depression in treating FD.There has more Clinical efficacy than the single use of anti-Helicobacter pylori treatment.
