精神分裂症患者治疗前后血清甲状腺激素及临床特征的动态变化

目的:探讨精神分裂症患者治疗前后血清甲状腺激素及临床特征的动态变化。方法:精神分裂症患者于入院时测定血清游离三点碘甲状腺原氨酸(FT3)、游离甲状腺素(FT4)及促甲状腺激素(TSH)水平,将结果异常者定为研究组(65例),结果正常者为对照组(313例)。治疗后第2、4、6、8周末测定两组患者的血清甲状腺激素水平,同时采用阴性症状和阳性症状量表(PANSS)及副反应量表(TESS)进行评定,并进行比较。结果:治疗前后研究组血清FT3值均显著低于对照组(P<0.05),治疗后两组血清FT4值均显著下降(P<0.05),但对照组下降较为明显(P<0.05),两组TSH值均明显增高。两组患者治疗前PANSS总分及各因子分无显著性差异,治疗后两组各分值均显著下降,但研究组PANSS总分下降不如对照组明显(P<0.05)。两组治疗后TESS分值无显著性差异(P>0.05)。结论:精神分裂症患者治疗后血清FT4值下降明显者疗效较好。在抗精神病药物治疗过程中,定期监测血清甲状腺激素水平,选择更合理、更有效的治疗方案。

The dynamic changes of the serum thyroid hormones and the clinic character of the schizophrenias before and after treatment

Objective:To discuss the dynamic changes of the serum thyroid hormones and the clinic character of the schizophrenia before and after treatment.Methods:To menstruate the serum FT3、FT4 and TSH of the Schizophrenia when they enter the hospital.To assume the novel results as the research group(65 cases),and the natural ones as the control group(313 cases).After treated 2 weeks,4 week,6 week,and 8week,menstruate the serum thyroid hormones of two groups.As the same time,we adopt the PANSS and the TESS to assess them,and then to compare them.Results: Before and after treament,the serum FT3 of the research group is both obviously less than the control group(P<0.05).After treated,the serum FT4 of two groups are both declined obviously(P<0.05).But the control group is declined more obviously than the research group(P<0.05).The serums TSH of two groups are both heightened obviously.Before treatment,the markings of the PANSS and apiece genes of both groups are no obviously difference.After treatment,all markings are obviously declined.But the markings of the PANSS of the research group declined less obviously than the control groups.About the markings of the TESS,two groups cases had no difference after treatment(P>0.05).Conclusions: To the schizophrenia,the serums FT4 declined more obviously,the effect is better after treatment.During the treatment,test the serum thyroid hormones,and then to select the more in reason and more effective RX.
