狼疮肾炎患者外周血淋巴细胞凋亡调控因子和血清尿白细胞介素-8的变化

摘    要:
目的探讨外周血淋巴细胞(PBL)凋亡调控因子和白细胞介素-8(IL-8)在活动性狼疮肾炎(LN)患者中的作用及其临床意义。方法对36例活动性LN患者PBL细胞凋亡调控因子Fas抗原、Fas配体、bcl-2和IL-8进行检测分析,并以15名正常自愿者为对照。Fas、FasL、bcl-2的表达水平运用免疫组织化学法测定。血清、尿IL-8用放射免疫法检测。结果活动性LN患者Fas、FasL、bcl-2表达均显著高于正常人(P<0.001),Fas、FasL表达与临床活动指数呈正相关(r=0.473、P<0.005;r=0.493,P<0.05),bcl-2表达与临床活动指数无显著相关性(r=0.063,P>0.05);血清、尿IL-8水平明显低于正常组(P<0.01),但与临床活动指数无关(r=0.038,P>0.05;(r=0.272,P>0.05),与Fas、FasL、bcl-2无明显相关关系。结论Fas、FasL、bcl-2的表达升高导致外周血淋巴细胞凋亡异常,可能为系统性红斑狼疮(SLE)、LN发病机制之一,PBL中Fas、FasL表达水平可作为病情活动的指标;血、尿IL-8水平低下,可能与免疫功能紊乱有关,有待进一步研究。

Abstract:
Objective To study the clinic significance of expressions of Fas/Fas ligand(FasL)and bcl-2in peripheral blood lymphocytes(PBL)and interleunkin-8(IL-8)in serum and urine from patients with active lu-pus nephritis(LN).Methods Thirty-six active LN patients were enrolled and15healthy volunteers were select-ed as controls.The expressions of Fas,FasL and bcl-2were examined by immunohistochemical staining.IL-8in serum and urine was evaluated by radiate immune assay.Results The expressions of Fas,FasL and bcl-2in PBL were significantly higher in active LN patients than those in healthy subjects(P<0.001).The level of IL-8in serum and urine was lower than that in normal control group(P<0.01).The expressions of Fas and FasL were posi-tively related to SLE disease active index(SLEDAI)(r=0.517?P<0.001;r=0.543,P<0.001)in the LN patients,but not to bcl-2(r=0.09,P>0.05)and IL-8in serum and urine(r=-0.04,P>0.05;r=0.43,P>0.05).There were no signifi-cant relation between the expressions of Fas,FasL and bcl-2and IL-8.Conclusion The abnormal expressions of Fas,FasL,bcl-2may play an important role in the pathogenesis of LN.The expression of Fas and FasL may serve as active indexes for SLE and LN.The low level of IL-8may be related to the dysfunction of immunity and needs further research.
