指尖冲压性断指再植体会

目的 介绍指尖冲压性离断的治疗效果和经验总结。方法 对85例103指指尖冲压性断指进行再植，吻合指固有动脉终末端或指动脉弓分支或指固有动脉终末端与指动脉弓分支吻合或动静脉转流，尽可能多吻合指掌侧静脉，对各类型平面的指尖离断的断指均进行再植。结果 各型成活率分别为：Ⅰ型83.9％，Ⅱ型72.5％，Ⅲ型57.1％。结论 只要冲压性离断的指体仍较完整，清创时能找到可供吻合的血管，应予以再植。高质量的血管的吻合技术和良好的静脉血回流是保证再值成功的关键。

Replantation of crushing severed fi ngertips

Objective To introduce the experience and the treatment outcome in repl a ntation of crushing severed fingertips. Methods One hundred and three cases crushing se vered fingertips of 85 patients have been replanted from July 1996 to September 2000. The anastomotic methods of arterial were as follow：anastomosis of the terminal branches of the digital artery，anas tomsis of the branches of the terminal digital arterial arch. The terminal branch es have been done with the branches of a rterial arch，and arteriovenous anastomos is.The superficial palmar digital veins were anastomosed.And replanted fingertips of all kinds have been studied.The post operative follow-up period was 1 to 27 months. Results The survival rate ：Ⅰ type 8 3.9％，Ⅱ type 72.5％，Ⅲ type 57.1％. Conc lusions Crushing severed fingertip could be replanted as long as the body of the crushing fingertip remained intact and the vessels for anastomosis could be found. Hight quality anastomosis and good venous return are crucial to the success of crushing fingertip replantation.
