rhGH治疗GHD患儿初期甲状腺功能变化及与疗效的关系

目的：探讨基因重组人生长激素（rhGH）治疗生长激素缺乏症（GHD）患儿初期对甲状腺功能的影响及甲状腺功能变化与疗效的关系。方法：在应用rhGH前及用药后第3、6个月做FT3、FT4及TSH检测；身高由专人于上午测量，采用同一人体测高仪，每次均测量3次取其均数值，计算出患儿3、6月时身高标准差计数及生长速率。结果：治疗前GHD患儿组甲状腺功能与正常对照组相比，FT3显著降低（P〈0．05），FT4与TSH无明显统计学意义；rhGH治疗3、6个月时与用药前相比FT3水平明显升高且具有统计学意义，治疗3个月后FT4与治疗前相比明显降低且具有统计学意义（P〈0．05），TSH治疗后呈下降趋势但无统计学意义（P〉0．05）；GHD组rhGH治疗后较治疗前生长速度（HV）增快且有统计学意义（P〈0．05），rhGH治疗后身高标准差计数逐渐升高且治疗6个月时与治疗前相比有统计学意义（P〈0．05）；治疗前后FT3与身高标准差计数及HV成正相关。结论：在rhGH治疗过程中应动态监测甲状腺功能的变化。

Thyroid function changes in children with GHD during the initial phase of rhGH replacemen therapy and the correlation between the changes and therapeutic effects
