HBV慢性感染肾组织免疫组化染色观察

目的重症HBV急、慢性感染时,出现肾功能衰竭,一直被认为属功能性,而器质性病理形态改变不明显,需进一步探讨。方法20世纪90年代起随机收集慢性乙肝、肝硬化和肝细胞癌病死者肝、肾组织作病理切片和免疫组化染色检查,计85例。结果85例肾组织病理切片镜检均见有轻重不等病理形态改变,免疫组化染色HBsAg和HBcAg阳性者高达75.3%。结论HBV慢性感染时,HBV可在肾组织内表达,同时肾组织亦存在轻重不等的病理形态改变,这可能是HBV感染者肾功能衰竭的基础,单纯功能性损害的看法值得商榷。

Survey on the immunohistochemical staining of renal tissue from chronic HBV infection

Objective It should be studied further that the hepatorenal syndrome of acute an d chronic HBV infection was considered as a functional disease without any obvious pathological changes in renal tissue so far.Methods The85specimens of renal tissues from chronic hepatitis,liver cirrhosis and hepatic carcinoma were collected at random for the study in recent 10years.Results All the85specimens had obvious abnormal pathological changes in microscopic examination and both HBsAg and HBcAg could be detected in75.3%specimens by immunohistochemical staining.Conclusion The facts,both exsistence and replication of HBsAg and HBcAg in renal tissues and obvious abnormal pathological changes in all the renal tissues,perhaps suggest that hepatorenal syndrome isn't a functional disease,but is basis of the renal failure.
