不稳定髌骨的X线测量及手术治疗的评价(附48例病例分析)

本文对48例(56个关节)临床诊断为不稳定髌骨的患者及50例作为对照的正常人作了髌股关节侧位及轴位X线测量研究。提出CA和LPFA两项指标的诊断意义最大,并根据CA和LPFA的测量结果,把不稳定髌骨力线异常分为单纯偏移、单纯倾斜和偏移倾斜三类。19例髌骨力线严重不良的患者,采用了E-T手术治疗,术后总优良率达78.9％。

ASSESSMENT OF X-RAY MEASUREMENT AND SURGICAL TREATMENT OF UNSTABLE PATELLAE (A REPORT OF 48 CASES)

Fourty-eight cases (56 joints) with unstable patella confirmed clinically, and 50 cases of asymptomatic volunteers were studied by axial roentgenography of the patello-femoral joint. CA (congruence angle)and LPFA (lateral patello-femoral angle)are more important in the diagnosis of unstable patella. Three distinct patterns of malalignment were identified according to the results of CA and LPFA: (1) lateralized, non-tilted patella, (2) tilted, non-lateralized patella, (3) lateralized and tilted patella. Nineteen patients with severe patellar malalignment were treated with the procedure described by Elmslie-Trillat. The result were excellent or good in 15 knees (78.9%).
