沙棘籽油防治急性放射性粘膜炎的临床疗效观察

目的：观察沙棘籽油对急性放射性口腔粘膜炎、放射性食管粘膜炎的作用。方法：可评价的100例头颈部和胸部肿瘤，其中头颈部肿瘤51例，分为粘膜预防用药组25例和对照组26例，预防用药组用沙棘籽油抹口腔咽部或口服，每日3次，每次5ml至放疗结束；对照组用生理盐水20ml，每日3次漱口。49例胸部肿瘤分为急性放射性食管炎治疗组25例，对照组24例，治疗组从出现放射性食管炎开始口服沙棘子油，每次5ml，每天3次，14～21天，对照组用生理盐水25ml＋20％利多卡因20ml＋地塞米松10mg溶液，每次10ml口服，每天3次，14～21天。两组患者均采用常规分割放疗。结果：预防用药组急性放射性口腔粘膜炎1级多出现在放疗20Gy左右，其发生率高于对照组。急性放射性口腔粘膜炎预防用药组与对照组比较，1、3～4级的发生率分别为64．0％、34．6％和8．0％、34．6％（P〈0．05）。急性放射性疼痛预防用药组1、2～3级分别为72．0％和28．0％，对照组分别为34．6％和65．3％（P〈0．01）。延迟放疗的例数预防用药组少于对照组（P〈0．05）。急性放射性食管粘膜炎治疗给药组的总有效率为92．0％，对照组为62．5％，两组之间差异显著（P〈0．05）。结论：沙棘籽油可降低急性放射性口腔粘膜炎的毒性分级，缓解急性放射性疼痛，对急性放射性食管粘膜炎的疗效好，有助于顺利完成放疗全过程。

Clinical study of seabuckthorn seed oil on acute radiation mucositis

Objective:To evaluate the effect seabuckthron seed oil on acute stomatitis and esophagitis induced by radiation.Methods:One hundred cases of head and neck and chest tumors were randomizedly divided into 2 groups.Mucositis pro-phylactic application(MPA)group with control were all of 51 cases.Two groups application plus concurrent radiotherapy were carried out.Esophagitis therapeutic application(ETA)group with control were 49 cases and therapeutic application was started as grade 1 radiation esophagitis.Two groups all were emoployed conventional fractionated radiation therapy.Results:The mean does of grade 1 acute radiation mucositis was to more than 20Gy in MPA group and the incidences was higher than incontrol group.The incidences of grade 1,3-4 acute radiation mucositis were 64.0%,34.6% and 8.0%,34.6% in MPA group and in control group(P<0.05).Grade 1 and grade 2-3 radiated pain occurened in MPA group and control group were 72.0%,28.0% and 34.6%,65.3%,respectively(P<0.01).Number of radiotherapy delays in MPA group were lower than those in control group,which was statiscally significant between two groups.The overall response rates of acute radiation esophagitis of ETA group and control group were 92.0% and 62.5%,respectively.Conclusion:Seabuckthron seed oil is be able to reduce toxicity grade of acute radiation mucositis and to relief radiated pain.The efficacy of acute radiation esophagitis is good for it.
