肺栓塞患者血浆D-二聚体水平与心脏超声指标间的关系研究

目的探讨肺栓塞(PE)患者D-二聚体水平与心脏超声指标间的关系。方法收集我院59例PE患者的临床检查资料结果,根据D-二聚体将入选患者分为两组:D-二聚体阳性组(≥0.5 mg/L);D-二聚体阴性组(<0.5 mg/L)。比较分析两组间相关临床检查资料主要指标之间的差异。结果两组在年龄、性别、下肢血栓形成例数、心电图典型改变(SIQⅢTⅢ)、肝功能、血脂、动脉血气分析等主要指标方面比较,差异无统计学意义(P>0.05)。D-二聚体阳性组的肺动脉平均压、室间隔受压变平或凸向左室比例高于D-二聚体阴性组,差异有统计学意义(P<0.05)。经过线性相关分析,D-二聚体与肺动脉平均压、右室舒张末内径之间不存在线性相关(P>0.05)。结论 D-二聚体阳性的PE患者肺动脉平均压、室间隔受压变平或凸向左室比例高于D-二聚体阴性者,但D-二聚体与肺动脉平均压、右室舒张末内径之间不存在线性相关关系。

Relationship between Plasma D-Dimer and Clinical Data Indexes in Patients with Pulmonary Embolism

Objective To review the relationship between D-dimer and other clinical data in 59 patients with pulmonary embolism(PE) diagnosed in our hospital.Methods Clinical data of 59 patients with PE were collected.According to D-dimer level the patients were divided as D-dimer positive group(≥0.5 mg/L) and D-dimer negative group(<0.5 mg/L).The differences in the two groups in main relevant clinical examinations were comparatively analyzed.Results No statistically significant difference between two groups was found in gender,age,thrombosis of lower limb,SIQⅢTⅢ in electrocardiograph,liver function test,blood fat,and artery blood gas analysis(P>0.05).There was statistically significant difference in mean pulmonary arterial pressure and interventricular septum transformation between the two groups(P<0.05),but there were not linear correlation between D-dimer and mean pulmonary arterial pressure,the end-diastolic inner diameter of the right ventricle(P>0.05).Conclusion The mean pulmonary arterial pressure and the ratio of interventricular septum transformation were significantly higher in D-dimer positive group than in D-dimer negative group,but there was not linear correlation between D-dimer and mean pulmonary arterial pressure,the end-diastolic inner diameter of right ventricle.
