斜视手术前后分视VEP的临床研究

目的:研究不伴弱视的斜视患者术前、术后分视VEP的变化,并探讨分视VEP检查评价双眼视功能的有效性。方法:1.斜视患者术前及术后进行同视机和Titmus立体图检查;2.利用同视机分视双眼,同时以黑白棋盘格翻转及静止图形刺激两只单眼,记录斜视患者术前、术后分视VEP;3.对照组进行同视机、Titmus立体图、分视VEP检查。结果:1.斜视患者术前分视VEP振幅低于对照组,两者差异有显著性(t=4.20,P<0.01)。不伴弱视的斜视患者术后分视VEP振幅较术前提高,两者差异有显著性(t=16.27P<0.01),斜视术后分视VEP振幅与对照组比较差异无显著性(t=0.152P>0.05)。斜视患者术前、术后及对照组分视VEP潜伏期差异无显著性(P>0.05);2.分视VEP检查结果与同视机、Titmus立体图所查斜视患者双眼视功能情况相符。结论:分视VEP检查是一种简单、客观有效的眼电生理检查法,分视VEP反应是一种更为精确的评价双眼视功能情况的双眼总和VEP反应。临床上这种方法与其他检查方法相结合可以更全面地了解斜视患者术前、术后双眼视功能的情况。

Effect of dichoptic VEPs in strabismus before and after surgery

Objective:To explore the postoperative changes of dichoptic visual evoked potentials(VEPs) in strabismus without amblyopia,and investigate its clinical significance on strabismus. Methods:Thirty-one caees of strabismus were recorded dichoptic VEPs elicited by reversed checkerboards and static gram preoperatively and postoperatively.30 normal subjects were examined by synoptophore,Titmus stereograph and dichoptic VEPs. Results: The difference of the amplitudes of dichoptic VEPs between normal subjects and strabismus preoperatively was significant(t=4.20,P< 0.01). The amplitudes of dichoptic VEPs were lower in strabismus than in normal subjects. After surgery, the amplitudes of dichoptic VEPs in strabismus were increased and had a significant difference with preoperative ones(t=16.27,P< 0.01),there was no difference of the amplitudes of dichoptic VEPs between normal subjects and strabismus postoperatively(t=0.152,P >0.05). There were no differences of the latentcy of dichoptic VEPs between normal subjects, strabismus preoperatively and postoperatively(P>0.05). Conclusion:The amplitudes of dichoptic VEPs in strabismus are increased postoperatively. The dichoptic VEPs can be performed quickly and easily on subjects and gives a quantitative assessment of cortical binocularity. This technique may also prove useful of the preoperative and postoperative gradation of binocular potentials.
