膝关节滑膜（骨）软骨瘤病的x线及MRI表现（附17例报告）

目的：分析x线及MRI对膝关节滑膜（骨）软骨瘤病的诊断价值。材料与方法：回顾性分析17例经病理证实的膝关节滑膜（骨）软骨瘤病的X线、MRI表现，比较两种影像学方法对本病的诊断价值。结果：X线平片9例关节间隙及关节周围可见高密度游离体并伴有关节软组织肿胀，6例表现为不同程度关节周围软组织肿胀，2例x线平片未见异常，x线平片的诊断正确率为53％。MRI检查16例表现为关节内游离体，1例表现为滑膜明显增生呈结节状，并可见滑膜内结节信号略低。17例患者均可见不同程度的滑膜增生及多少不等的关节积液，MRI的诊断正确率为94％。结论：X线平片检查是诊断膝关节滑膜（骨）软骨瘤病最简便方法，而MRI检查对于早期病例及术前病例有更高的价值。

X-ray and MRI Findings of Synovial Chondromatosis of the Knee Joint （Report of 17 Cases）

Purpose： To analyze the value of X-ray and MRI in the diagnosis of synovial Chondromatosis of the knee joint. Materials and Methods： The X-ray and MRI findings of 17 eases of pathologically confirmed synovial Chondromatosis were analyzed retrospeetively. The value of the two imaging methods for the diagnosis of this disease was compared. Results： On X-ray film, flee bodies of high-density could seen in or around the joint space accompanied by joint and soft tissue swelling in 9 eases, showed varying degrees of articular soft tissue swelling in 6 eases, 2 eases showed no abnormality. The diagnostic accuracy of X-ray was 53%. On MRI, intra-articular free bodies were showed in 16 cases, obvious nodular hyperplasia of syn- ovial was showed in 1 case. Varying degrees of synovial hyperplasia and varying amounts of joint effusion were seen in all 17 eases. The diagnostic accuracy of MRI was 94%. Conclusion： X-ray film is the basic method for the diagnosis of knee syno- vial ehondromatosis, and MRI have a higher value before surgery and for early diagnosis.
