小骨窗开颅内镜辅助治疗自发性脑出血12例

目的 探讨内镜微创手术治疗自发性脑出血。方法 采用小骨窗开颅内镜辅助手术治疗自发性脑出血患者12例。结果 血肿完全清除7例，大部清除4例。术后再出血1例，2例为血管畸形。随访3～6个月，GOS分级良好3例；中残4例；重残5例；无一例死亡。结论 内镜手术清除自发性脑出血手术创伤小，能在直视下操作。对小血管畸形导致自发性脑出血也可应用内镜微创手术治疗。

Primary application of endoscope assisted small bone window craniotomy for treatment of spontaneous intracerebral hemorrhage of 12 cases

Objective To explore minimal invasive endoscopic operation for treatment of spontaneous in-tracerebral hemorrhage. Methods Twelve cases of spontaneous intracerebral hemorrhage were evacuated with endoscope-assisted small-bone-window craniotomy. Results Complete evacuation of hematoma were achieved in 7 cases; partial evacuation in 4 cases; and rebleeded in 1 case. All patients were followed up for 3 to 6 months. According to GOS, good recovery were achieved in 3 cases, moderate disability in 4 cases, severe disability in 5 cases, and there was no death. Conclusion Endoscopic operation for spontaneous intracerebral hemorrhage is a minimal invasive operation which under direct vision, and also can be used for intracerebral hemorrhage caused by small vascular malformation.
