非霍奇金淋巴瘤骨髓侵犯69例临床分析

摘    要:目的:探讨非霍奇金淋巴瘤(NHL)骨髓侵犯患者的临床特点、病理类型、细胞形态特征、治疗效果与预后。方法:对404例经病理学确诊为非霍奇金淋巴瘤患者行血常规,骨髓细胞学检查,分析其骨髓侵犯发生率、细胞形态特征、病理类型、临床表现、分期、疗效及预后的关系。结果:例患者中骨髓侵犯69例.404NHL(17.1%),形态学特征与原发肿瘤一致,年龄35岁以下者多见(53.6%),病理类型以B细胞淋巴瘤为主(78.2%),骨髓侵犯晚期多见,其侵犯程度越重,临床症状越重,化疗效果差,缓解率低,预后不良。结论:NHL骨髓侵犯程度与临床症状严重程度、病理类型、治疗与预后等密切相关,骨髓侵犯程度重,临床分期晚,疗效及预后差。

Abstract:Objective:To investigate the clinical features, pathologic type, cyto - morphological (bone marrow smear) features, therapeutic efficacy and clinical prognosis of bone marrow invasion of non - Hodgkins lymphoma (NHL) patients. Methods :The study included 404 cases diagnosed as NHL by pathology. Their morphological char- acteristics, degree of bone marrow invasion ,pathologic type, clinical features ,curative effect ,prognosis were analyzed. Results :The incidence of bone marrow invasion was 17.1%, the morphological characteristic was just as same as the primary tumor. The patients under 35 years old was 53.6%. Bone marrow invasion was common in B - cell lymphoma (78.2%). The more serious the bone marrow was invaded,the more serious the clinical symptoms was ,the poorer the remission rate of chemotherapy was, the worse the curative effect was. Conclusion:The degree of invasion was correlated significantly with the clinical features, pathologic type, curative effect and prognosis.
