非霍奇金淋巴瘤骨髓侵犯69例临床分析

目的：探讨非霍奇金淋巴瘤（NHL）骨髓侵犯患者的临床特点、病理类型、细胞形态特征、治疗效果与预后。方法：对404例经病理学确诊为非霍奇金淋巴瘤患者行血常规，骨髓细胞学检查，分析其骨髓侵犯发生率、细胞形态特征、病理类型、临床表现、分期、疗效及预后的关系。结果：例患者中骨髓侵犯69例．404NHL（17．1％），形态学特征与原发肿瘤一致，年龄35岁以下者多见（53．6％），病理类型以B细胞淋巴瘤为主（78．2％），骨髓侵犯晚期多见，其侵犯程度越重，临床症状越重，化疗效果差，缓解率低，预后不良。结论：NHL骨髓侵犯程度与临床症状严重程度、病理类型、治疗与预后等密切相关，骨髓侵犯程度重，临床分期晚，疗效及预后差。

Clinical analysis of non -Hodgkins lymphoma with bone marrow invasion in 69 cases

Objective：To investigate the clinical features, pathologic type, cyto - morphological （bone marrow smear） features, therapeutic efficacy and clinical prognosis of bone marrow invasion of non - Hodgkins lymphoma （NHL） patients. Methods ：The study included 404 cases diagnosed as NHL by pathology. Their morphological char- acteristics, degree of bone marrow invasion ,pathologic type, clinical features ,curative effect ,prognosis were analyzed. Results ：The incidence of bone marrow invasion was 17.1%, the morphological characteristic was just as same as the primary tumor. The patients under 35 years old was 53.6%. Bone marrow invasion was common in B - cell lymphoma （78.2%）. The more serious the bone marrow was invaded,the more serious the clinical symptoms was ,the poorer the remission rate of chemotherapy was, the worse the curative effect was. Conclusion：The degree of invasion was correlated significantly with the clinical features, pathologic type, curative effect and prognosis.
