腰椎管狭窄症的手术治疗（附50例报告）

目的：探讨腰椎管狭窄症的手术治疗效果。方法：50例腰椎管狭窄症患者经手术治疗，34例行椎管后壁切除术，侧前方人路行腰椎间盘或腰椎后纵韧带骨化病灶切除12例，后路环椎管减压术4例。功能评定采用改良的Epstein评分标准，结果分为优、良、可、差四级。结果：本组50例术后随访5个月～37个月（平均16±3．2个月），结果：优27例，良14例，改善9例，差0例，优良率为82％。结论：手术是治疗腰椎管狭窄症的一种比较有效的方法。

Treatment of lumbar spinal stenosis by surgery (50 cases report)

Objective： To explore retrospectively the surgical effect of lumbar spinal stenosis. Method： Fifty patients were trea- ted by surgical protocol with lumbar spinal stenosis between July 2000 and March 2009. The 34 cases were performed decompression of the posterior wall of the lumbar spinal canal at all affected levels with predominant posterior compression of the neural elements consis- ted. 12 cases were underwent anterior decompression of the lumbar canal due to the predominance of anterior pathology. 4 cases circles spinal canal decompression. The clinical outcomes were graded into excellent, good, fair and poor basing on the Epstein grade system Result： After an average duration of 16 ±3. 2 months of follow -up ranging from 5 to 37 months, the outcomes were excellent in 27 cases （27/50, 54% ）, good in 14 cases （14/50, 28% ）, fair in 9 cases （9/50, 18% ） and poor in 0 cases （0/50, 130. 00% ） out of the 50 cases, and the rate of both excellent and good were 82%. Conclusion： The surgical protocol is an effective method to treat the lumbar spinal stenosis.
