锁骨钩钢板与克氏针内固定治疗肩锁关节脱位的疗效评价

目的：对比分析应用锁骨钩钢板（CHP）与克氏针内固定术（TBF）治疗肩锁关节脱位的临床疗效。方法：回顾性分析2008年1月至2011年12月因患有肩锁关节脱位于我院行手术治疗的96例患者临床资料，其中行克氏针内固定术治疗患者46例，锁骨钩钢板固定治疗患者50例，术后平均随访一年（9—32个月进行随访，平均为12个月）。根据Karlsson标准进行肩关节功能恢复评定。观测两组术后优良率及并发症感染率并进行统计学分析。结果：CHP组的优良率为（98．00％）明显高于TBF组的优良率（71．74％），并发症发生率为（2．00％）明显低于TBF组（39．13％），差异有统计学意义（P〈0．05）。结论：锁骨钩钢板治疗与克氏针内固定治疗肩锁关节脱位相比具有优良率高。术后并发症机率小少，恢复快，疗效确切等有优点，是治疗肩锁关节脱位（TossyⅢ型）较合理的治疗方式。

Curative Effect Evaluation of Clavicular Hook Plate and Kirschner Wire Fixation in the Treatment of Acromioclavicular Joint Dislocation

Objective： Comparative analysis of the effect of clavicular hook plate （ CHP ） and Kir- schner wire internal fixation （ TBF ） for the treatment of acromioclavicular joint dislocation. Method： A ret-rospective analysis operation treatment, from Jan. 2008 to Dec. 2011 due to suffering from the acromiocla-vicular joint dislocated in our hospital, 46 cases the line Kirschner wire internal fixation for the treatment,50 patients with clavicular hook plate fixation, after a mean follow-up period of one year （ 9-32 months follow -up, for an average of 12 months ）. According to the criteria of Karlsson recovery of shoulder function as- sessment, observed excellent and good rate and complication rate of infection and made a statistical analysis. Result： In CHP group, the excellent and good rate was 98 % , which was significantly higher than that in TBF group （ 71.74% ） , the excellent and good rate of complication rate （ 2% ） was significantly lower than that in TBF group （ 39.13% ）, the difference was statistically significant （ P〈0.05 ）. Conclusion： Trea- ted with clavicular hook plate and Kirsehner wire internal fixation for tile treatment of dislocation of acromio- clavicular joint has excellent high rate, low probability of less postoperative complications, quick recovery, curative effect is exact and other advantages, is the treatment of dislocation of the acromioclavicular joint （ TossyⅢ） is a reasonable treatment.
