我院Ⅰ类切口围术期预防用抗菌药物不合理使用情况分析

目的分析我院普外科在围术期预防用抗菌药物中存在的不合理现象,以促进临床预防用抗菌药物的合理性。方法对2010年1季度检查中发现的117例Ⅰ类切口围术期预防用抗菌药物不合格病历进行分类汇总,按抗菌药物的种类、术前术后使用的时间、是否存在出院带抗菌药物等进行分析,从而筛选出不合格的主要因素。结果被调查的不合格病历中,用药时间超过24 h的共有85例,占28.1%。术后继续使用抗菌药物的为71例,占23.5%。抗菌药物品种选择不当的45例,占14.9%。违规出院带抗菌药物的为44例,占14.6%。未见有两联及以上使用抗生素病历。结论围术期预防用抗菌药物时间超过24 h、术后继续使用抗菌药物、抗菌药物品种选择不正确及出院带抗菌药物是导致Ⅰ类切口围术期抗菌药物使用不合格的主要因素,要规范围术期用药,需要在这几个方面进行重点干预。

Analysis on irrational usage of prophylactic antibacterial in type I incision opera- tion

AIM To analyse the masons of prophylactic antibacterial in type I incision operation in order to pro- mote rational use of antibacterial in perioperative period. METHODS Analysis on 117 case files which with irrational use of prophylactic antibacterial in type I incision operation, all data were classified by type of antibacterial and course of treatment, etc. RESULTS These data suggested that 81.1% disqualificated case files were concentrated on the course of treatment exceed 24 h（85,28.1% ）, continuous administration of antibacterial（71, 23.5 % ）, using of unsuit- able type of antibacterial（45, 14.9%） and discharge with oral antibacterial（44, 14.6% ）. CONCLUSION To control irrational usage of antibacterial in perioperative period effectively we need to emphasize on the proper use of antibacterial and strength the antibacterial management.
