Pentacam对准分子激光原位角膜磨镶术术后角膜后表面的临床分析

摘    要:目的利用Pentacam分析准分子激光原位角膜磨镶术(LASIK)手术前后角膜后表面高度及形态变化。方法随机收集在青岛眼科医院行LASIK的61例(122只眼)患者,按屈光度分组:A组(〈-3.00 D),B组(-3.00 D~-6.00 D),C组(-6.00 D~-10.00 D),D组(〉-10.00 D),按残余基质床厚度占术前角膜厚度的比例(即切削比)分组:E组(〈50%),F组(50%~55%),G组(〉55%)。手术采用Zyoptchix XP旋转角膜板层切削刀,用Pentacam测量术前及术后1个月、3个月的角膜形态,对比不同分组手术前后角膜后表面形态及高度变化。结果术前和术后角膜后表面形态多不完全桥型和桥形递减型,为62.9%、50%和33.9%、31.9%,术后中央岛和桥形递增型为8.9%和4.8%,多在屈光度〉-6.00 D中;不同屈光度组与不同切削比组手术前后角膜后表面高度组间比较无统计学意义(P=0.324,P=0.476),术后1个月、3个月组间比较均有统计学意义(P〈0.01),屈光度大于-6.00 D与切削比小于55%时组内比较术前与术后角膜后表面高度有统计学意义(P〈0.05)。结论Pentacam分析LASIK术后角膜后表面前凸主要在屈光度〉-6.00 D中,切削比控制在55%以上时角膜后表面前凸度减小明显。
